On February 2, 2009, the company filed for reorganization under Chapter 11 of the federal Bankruptcy Act, case number 09-11191-PGH. This permits the company to continue to operate with court supervision while developing a plan of reorganization. Those with monetary claims against the company are advised to obtain a Proof of Claim form by writing to Florida Southern District Court CM/ECF, 1515 N. Flagler Dr. Room 801, West Palm Beach, FL, 33401 for resubmission to that court. The case number should be entered on the form. Claims must be submitted by June 29, 2009.
